When a supplier needs UVA's IRS Substitute W-9 Tax Information, Request for Taxpayer Identification Number W-9 information:
- Substitute W-9 Form IRS W-9 with Dunn number
- Substitute W-9 Form IRS W-9 with Unique Entity Identifier (UEI)
